A minor issue is that the questionnaire template does not support Rich Text at the top (most scales have a particular heading with instructions and these need to be replicated as closely as possible [including bold, italics, underlying, paragraphs], otherwise the score on the paper questionnaire and electronic questionnaire may diverge. Importantly many scales do not have the name of the scale in the title so what the scale is called and the text at the top are independent. A few scales have text between some items for example the first few items might refer to the last 30 days and the remaining questions to the last 7 days.
